Anonymous Survey

This form will allow you to anonymously submit a comment or concern to the current DOC’s email if you feel uncomfortable reaching out in person.

When making a complaint you must complete all of the questions and fields for it to be considered and acted upon. Not providing enough information will result in the survey being dismissed.

When submitting something to the form remember our bylaws and code of conduct. Explaining a situation and your feelings about the situation is fine - hateful language and name calling is unwelcome.

Please use this form for broad, systematic complaints such as problems with practice structure, BOD conduct, or complaints about overall league policies. For complaints that will require mediation/investigation such as issues of misconduct by league members please email bod@crowcityderby.com. We promise to remain impartial and keep conversations within the BOD in order to protect the anonymity of any member who approaches us, as per our BOD code of conduct.
